Waterfall Failure Logic -- Last week, I introduced you to Clarke Ching. He isn't embarrassing me, to say the least. In the subject link, he's included a couple of what we in TOC-land call "Current Reality Trees" (CRTs) that describes the logic of undesirable effects of hard phase-defined "waterfall" plans. A very nice analysis -- both from the point of view of the subject and as an example of a CRT. For me it raises a question about the assumed definition of "phase." It'll be interesting to watch his solution unfold.
